Gallai\u2019r meddyg fod yn arbenigwr clinigol ar Hepatitis C (naill ai\u2019n hepatolegydd neu\u2019n ymgynghorydd ar glefydau heintus) neu gallai fod yn ymgynghorydd arall os bydd yn ymwneud \u00e2 thrin eich salwch.<\/p>\n<p>Pan fydd Cynllun Cynorthwyo Gwaed Heintiedig Cymru wedi ystyried eich ffurflen, os cadarnheir eich bod yn gymwys i gael cymorth Hepatitis C datblygedig, cewch eich cyfandaliad ychwanegol a bydd eich taliad rheolaidd yn dechrau cyn gynted \u00e2 phosibl, gyda manylion personol i chi\u2019n cael eu hanfon trwy eich dewisiadau cysylltu. Bydd eich taliadau rheolaidd yn cael eu h\u00f4l-ddyddio i\u2019r dyddiad pan gyflwynoch eich cais, ar sail pro rata ar gyfer y flwyddyn ariannol.<\/p>\n<p><strong>Nod Cynllun Cynorthwyo Gwaed Heintiedig Cymru fydd rhoi penderfyniad ar bob cais o fewn 25 diwrnod gwaith, er y gall fod yn hirach os byddant angen gofyn i chi neu eich meddyg am wybodaeth ychwanegol.<\/strong><\/p>\n<p>Cysylltwch \u00e2 Gwasanaeth Cynorthwyo Gwaed Heintiedig Cymru os oes gennych unrhyw ymholiadau, os nad ydych yn si\u0175r pa ffurflen i\u2019w llenwi neu os hoffech help i lenwi\u2019r ffurflenni.
